You are the professional Social Work Coordinator we are looking for our River Bend Hospital location in West Lafayette, Indiana. The Social Work Coordinator is a therapist at River Bend Hospital who is part of the multidisciplinary psychiatric team, providing social services on the inpatient unit.

Social Work Coordinator Key Responsibilities:

  • Provides assessment, case formulation, diagnosis, and treatment planning to clients in the assigned service. This position will be solely responsible for intake assessment, treatment plan review and treatment plan worksheet within 60 hours of admission.
  • Provide coaching to nursing staff as needed for treatment plan.
  • Input and update all patient diagnoses from physician’s documentation in a timely manner.
  • Submit diagnoses for billing from physician’s documentation in a timely manner at intake and discharge.
  • Assist with formulation and submission of legal documentation to court for the purpose of client commitment and EDO. Social Work Lead will be solely responsible for ensuring the following reports are completed (commitment filings, hearings, inputting legal notes into the EMAR, notifying courts when patient is discharged, be the liaison for court needs and communication.
  • Training, coaching for social work assistants on appropriate documentation for social work assessments and group.
  • As needed provide shadowing and training for nursing/nurse tech staff on group’s run.
  • Provides and/or supervises an appropriate range of individual therapy modalities.
  • Provides and/or supervises the delivery of group therapy.
  • Provides and/or supervises the delivery of family/couple therapy.
  • Provides crisis/emergency services, as needed.
  • Provides case management/discharge planning. Initials discharge instructions and works closely with Nursing staff to complete discharge instructions.
  • Collaborates with and assists primary caregivers to meet the needs of the client.
  • Moves clients through the appropriate treatment with expected outcomes.
  • Deals positively with consumer and family requests.
  • Prepares the client and family for transfer/discharge.
  • Maintains professional boundaries.
  • Attends team meetings and staff meetings.
  • Meets with the assigned supervisor for clinical and administrative supervision.
  • Good time management skills including the ability to structure time, prioritize activities, and be self-motivated.
  • Works well with the staff of other agencies and with colleagues.
  • Effectively works as a member of the treatment team.
  • Effectively communicates with consumers, families, coworkers, and agency staff.
  • Teaches, trains, and/or supervises students, as assigned.
  • Maintains client’s records in accordance with Hospital standards.
  • Applies Hospital quality data, new therapy techniques, and research to improve client services.
  • Retains competencies/privileges as required for this position and membership in the appropriate discipline of the Professional Staff Organization.
  • Continues professional growth and development through participation in in-service training, professional groups, specialized workshops, and individual study.
  • Participates in quality improvement activities including committees, task groups, PIT teams, as assigned. Will be solely responsible for social work quality and improvement tracking and formulate presentation with assistance from Director for committee meetings.
  • Assist with editing policy for Social work director as needed and provide training of updates to policy to social work students/nursing staff and other staff.
  • Provides other duties, as assigned.

We are seeking Social Work Coordinator candidates with the following qualities:

  • Master’s degree in Social Work from a graduate school accredited by the Council on Social Work Education, or Master’s Degree in Marriage and Family Therapy or Counseling from an accredited counseling/therapy program accepted by the Indiana Health Professions Bureau with 60 credit hours of graduate work and the required practicum acceptable for licensure in Indiana.
  • Social workers must maintain an Indiana License as a Social Worker or Licensed Clinical Social Worker, as eligible. Mental Health Counselors must maintain an Indiana License as a Mental Health Counselor, as eligible. Marriage and Family Therapists must maintain an Indiana License as a Marriage and Family Therapist, as eligible. License must be non-probationary.
  • Two years relatable experience.
  • Able to achieve the required competencies or privileges with Meridian Health Services/River Bend Hospital .
  • Holds a valid Indiana Driver’s License, has a reliable vehicle, a good driving record, and automobile insurance.
